4. For this assignment, you will reflect on an older adult client (60 years or older) you have seen as a PMHN student in the psychiatric clinical setting, or create a fictional brief case study. You will identify unique client problems among older adults and amenable to inter-professional consultation or collaboration. Do not include the client’s identifying information.
5. Refer to your literature search and review as you develop your discussion post.
6. Compose an Initial post consisting of at least 500 words, addressing the following items:
   1. What is the client’s chief complaint and clinical presentation, including the DSM-5 supported mental health diagnosis?
   2. Which of this client’s problems, that you and/or the client have identified, cannot be met sufficiently by the PMHNP, alone?
   3. Based on the identified client problems, which interprofessional discipline or clinician can you consult with to help meet client needs?
   4. Discuss your evaluation of the results of this collaborative process – for example, how will you know if the desired outcome has been met, or – did the consultation lead to a positive client outcome?
