It is beneficial for an individual’s overall (physical, mental, social) well-being to have access to a range of services and facilities. For example supporting an individual to use the bus helps them to feel part of the society, doing something that others do. They can also meet others which helps them to socialize.

Environmental: lack of ramps, disabled access/toilets, lifts, high risk locations.

Communication: no translators, lack of loop system, poor communication skills, lack of information or publicity about the service / facilities.

By gathering as much information as possible about the services and facilities, then the issues, risks and hazards get assessed before creating a plan alongside the individual to help overcome discrimination and barriers.

I’ve noticed that Mr G., who has his meal delivered by Forerunners always having lamb for dinner. When I asked why, he said because he has got problems with chewing and lamb is the easiest to chew for him. I’ve told him that he can request Forerunners to liquidize his meal so he can order different kind of meals. Since then he is very pleased that he can have a variety to choose from.

The best way to do so is to listen to individuals’ concerns as each one may have a totally different and genuine need. It would also be beneficial in encouraging friends, family, other users of facilities and services to listen to the individual because everyone has a right to voice their concerns and have their voice heard.

I work with individuals to agree preferred options for services and facilities, identifying any risks that need to be managed and inform other healthcare professionals to select the services and facilities that will enable them to meet the individuals’ assessed needs.

The best way is to encourage individuals to give feedback. We have to ensure them that if they are not happy with the service there are still plenty of other solutions to meet their needs.

I would collect all the information and record/report all the actions, procedures and outcomes to the line manager.

4.4Identify and agree any changes needed to improve the experience and outcomes of accessing and using services or facilities

For example Mr R’s grab rail handle was not fitted at the right place. It was just not comfortable for him to grab and have a shower at the same time. We had to contact his occupation therapist to reassess him and agreed to be refitted.
